Sports Physicals:
All students who wish to participate in a school sport must have a physical on file in the nurses' office before tryouts or practice. Physicals are good for 13 months from the date of the physical. Students must present the physical to the nurse not to the coach. Please see below for a physical form.
10th Grade Physicals:
As per state law and Enfield Public School Policy, all students in 10th grade are required to have a health assessment completed and on file with the school nurse. This assessment must be conducted and submitted after the student’s last day of freshman year but before the first day of the student’s junior year. The 10th grade physical must be completed on the State of Connecticut blue Health Assessment form.
Secondary Schools Medical Update:
During the school year, please keep the nurse updated on your child's current medical issues. We also appreciate having current phone numbers available in order to contact you.
Medications:
Students are not allowed to carry any type of medication (except asthma inhalers or an Epi-pen at the secondary level). Students who need to carry their inhalers or have medications in the nurse’s office need to have the proper forms on file with the nurse.
